Merkel cell carcinoma is the correct diagnosis although immunohistochemistry is obviously necessary to confirm it. Two tumors which should be particularly excluded are small cell melanoma and T-cell lympomas since Merkel cell carcinoma sometimes shows epidermotropism.

Undoubtedly malignant tumor. My first impression is neuroendocrine tumour. Either primary in the skin, or a metastasis (?lung).
