I would also like to welcome Richard to my website. He is an excellent dermatopathologist and I know that his contribution will enhance the site greatly. Welcome on board Richard. The diagnosis is sebaceous carcinoma. The one thing you didn't mention is that it appears to be an in situ lesion which is common in ocular lesions but very rare in extra-ocular variants.

I agree with Sebaceous Carcinoma. I would like to extend a warm welcome to Dr Richard Carr who will be joining Phillip McKee in presenting cases for the Spot Diagnosis on a regular basis. Dr Carr will be managing the Friday spot diagnosis and will also provide cover when Phillip is on vacation. We are grateful to him for taking the time to contribute to this educational site.
