Your are all spot on. The IMF was inconclusive in this case but histologically and clinically we favoured bullous pemphigoid. There was an excellent response to systemic steroids and the rash started to re-appear when the dose of prednisolone was reduced to 5mg. Based on clinicopathological correlation our diagnosis is bullous pemphigoid. It is important to remember that eosinophilic spongiosis is a reaction pattern and not a diagnosis. Remember IMF like nearly any other investigation is not 100% sensitive.

Subepidermal eosinophil rich bullous disease---Bullous pemphigoid. There is reepithelialization of the floor of the bulla in areas giving a false impression of being intraepidermal. IF is required to confirm Bullous pemphigoid.
