Female 32 years with a history of widespread erythematous macules some with a central papule and/or vesicle formation.  Oral lesions were also present.

Given the acrosyringeal involvement, could this be drug-related EM (as opposed to herpes-related)?

diffuse interface dermatitis, c/w EM in the clinical setting noted. However, the parakeratosis seems out of sync w/more acute changes seen, so I will include ddx: drug-induced EM, other drug (including early FDE), early GVHD, early Mucha-habermann

Well done everyone. This is a nice example of erythema multiforme. The differential diagnosis that Marie has given is good. The absence of any eosinophils makes a drug-related example fairly unlikely although it doesn't exclude it. Graft-versus-host disease can be indistinguishable from the histology shown in this case. Subacute cutaneous lupus erytematosus would be another histological differential diagnosis

Website news. We are in the process of adding some new features to the site including a module called "Dermatopathology for beginners". This will be in very short video format and will briefly discuss basic terminology such as acantholysis- what it means, what it looks like and what conditions one can see it in.
New modues on specific topics such as nail disease and alopecia are being prepared and there will be more full length lectures on intersting topics such as mycosis fungoides etc. We plan to also have full length mock examinations for those sitting the Boards in Dermatopathology and the Diploma in Dermatopathology.
