Clear cell hidradenoma. Stroma is not richly vascular to suggest metastatic clear cell RCC.

I don’t believe this lesion is clear cell hidradenoma because the tumor has a trabecular rather than a solid nodular pattern. Furthermore, there’s no cuboidal-lined ductal structures, but foci of necrosis are present in picture #1.

While metastatic RCC is an important and consequential differential diagnosis I favor clear cell hidradenoma pending IHCs. Metastatic renal cell carcinoma as per Lever’s histopathology of skin occurs almost always in men. There is no significant extravasation of red cells and nuclei are very bland. There also appears to be a transition from more basaloid cells in the periphery to clear cells in the center.

I worry when I see a clear cell hidradenoma which is a bit vascular and i can understand why colleagues have gone for RCC. However the age and the lack of vascular prominence, would be against the latter.

Clear cell hidradenoma is correct. This is a classical example. Arif- Lever!! I think you need to buy a better book!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
Remember as there are only 4 images so I don't always show all the features. There were plenty of ducts in the section. The absence of hemorrage and limited vascularity are against metastatic RCC as is the marked uniformity of the tumor cell nuclei.
