Metastatic carcinoma with prominent epidermotropism mimicking paget's disease. I favor breast lobular carcinoma as a primary site. Immuno for CK7, CK20, TTF1, ER, GCDFP15, CEA, E-CADHERIN AND CD10 is required. DD includes metastatic signet ring carcinoma.

Agree with epidermotropic metastatic carcinoma. Although indian filing suggests lobular breast carcinoma, there is residual tubule formation and the neoplastic cells are too pleomorphic. So I think the primary tumor is ductal breast carcinoma.

Well done. This is a metastasis with epidermotropism or Paget's disease if you like. I am not sure but agree that it certainly looks like breast would be a likely source for the primary. Have a wonderful Christmas. The spots will resume on December 26th.
