Nephrogenic systemic fibrosis vs scleromyxedema. I favor the former due to involvement of subcutis. Is there a history of renal insufficiency??

Agree with Dr. Hamed. In addition to history a CD34 stain may also be helpful.

I am very impressed Hazem and Arif. You are absolutely correct. The diagnosis is nephrogenic systemic fibrosis and the patient did have a history of renal failure with dialysis. There will not be a spot diagnosis tomorrow. Happy New Year to you all.
