Fibroepithelioma of Pinkus was the diagnosis rendered in this case. It is very hard to get rid of old names but I agree with Eduardo and Richard that this lesion most probably represents a variant (fenestrated) of trichoblastoma although sometimes very BCC-like areas may be a feature. I have not seen an example associated with an invasive malignant tumor

My personal view is that fibroepithelioma of Pinkus is distinct from BCC (i.e. a fenestrated trichoblastoma) but clearly not everyone aggrees. In 10% of Pinkus lesions there are foci morphologically indistinguishable from BCC but as far as I am aware no case has been shown to be truely destructively invasive within the reticular dermis. No doubt occasional cases my transform to malignancy as we have seen in other trichoblastomas (rarely).

At the advanced dermatopathology course, Dr. Calonje mentioned that fibroepithelioma of Pinkus is probably a form of trichoblastoma since the lesions are mainly observed in lower back (relatively UV-protected area) and a densely cellular stroma is common. The abundant densely cellular stroma in this case (just like in trichoblastoma) supports his comments at this point.
