This is a case which depends heavily on clinicopathological correlation and immunohistochemistry. The differential diagnosis is broad with mycosis fungoides, pagetoid reticulosis, aggressive CD8+ epidermotropic T-cell lymphoma being among the leading contenders. An MF-like drug reaction is also a good thought. The actual diagnosis is pagetoid reticulosis- a solitary plaque on the foot is a typical site

MF however, drug induced T cell pseudolymphoma can mimic MF clinically and pathologically. Careful drug history is mandatory. IH and TCR gene re-arrangement studies are needed to confirm.
