There is follicular spongiosis with lymphocytosis and mucin deposition seen in Fig 4. The diagnosis is follicular mucinosis. This case was associated with mycosis fungoides. Eosinophils are often seen in this condition. I differentiate between folliculotropic MF and follicular mucinosis (alopecia mucinosa) since the clinical presentations are quite different and there is absence of mucin in folliculotropic MF.

DENSE DERMAL INFILTRATE OF LYMPHOCYTES, HISTIOCYTES AND EOSINOPHILS. IN AREAS, GRANULOMATOUS INFLAMMATION IS NOTED. I THINK LEPROSY SHOULD BE CONSIDERED. THE DIFFERENTIAL DIAGNOSIS OF GRANULOMA FACIALE ALSO REQUIRES CONSIDERATION.

Dense dermal infiltrate of lymphocytes, histiocytes with eosinophils. Infiltrate superficial and deep.Do not see neutrophils or nuclear dust though there is a sort of grenz zone on low power (was considering granuloma faciale but nuclear dust or evidence of vasculitis seen; though there is some endothelial swelling- ? reactive). However the epidermis does not appear normal and there does appear to be some spongiosis. Would like to see the epidermis up close. Given the features I think PAS and gram stain would be helpful to rule out an infective etiology. The other differential is granulomatous rosacea. CPC needed.
