Squamous carcinoma with CLL is correct. The patient was known to have CLL. This is a phenomenon which one sees from time to time. Occasionally, the clinician is unaware of the CLL and so you can score a diagnostic coup if you recognise the lesion. Lymphoepithelioma-like carcinoma consists of clusters and nests of highly malignant cells with a surrounding infiltrate. In this case, the squamous nature of the tumor is readily seen.

I will consider this SCC in a patient with chronic lymphocytic leukemia, small B cell lymphoma, because the epithelial component is showing features of squamous differentiation (dyskeratosis and foci of keratinization), features not seen in lymphoepithelioma like carcinoma.
