This is a very difficult case and I certainly would have entertained Merkel cell tumor, PNET etc as likely diagnoses. In fact it is from a patient with acute myelomonocytic leukemia and represents a leukemic deposit. Just goes to show you have to have a very open mind. I personally find leukemic infiltrates very difficult to recognise unless I think of it!!!!!
