First of all thank you Iskander and Richard for looking after the spot diagnoses while I was away. It is much appreciated. Yes, the correct diagnosis is hypopigmented mycosis fungoides. An MF-like drug reaction is always a good histological differential diagnosis. I remember one patient who had been diagnosed as MF by the clinicans and when I looked at the sections, I mentioned a drug reaction which is what it turned out to be!!

Hypopigmented MF. Lymphomatoid drug reaction should be excluded.

Hypopigmented mycosis fungoides. Interesting point is the lymphoid proliferation is CD4-/CD8+ in majority of cases despite excellent prognosis (Sezer E, Sezer T, Senayli A, Koseoglu D, Filiz N. Hypopigmented mycosis fungoides in a Caucasian child. Eur J Dermatol. 2006 Sep-Oct;16(5):584).

Epidermotropism. It seems a Mycosis Fungoides. Hypopigmented variant is more common in younger patients.
