There is dermal mucin deposition which extends in between the muscle fibers. Though it can be seen in various conditions (neoplastic or non neoplastic), I think there are no additional features of possible underlying cause. It does not look like mucocele (no cyst or granulation tissue). l would think of sclerdema and thyroid myxedema. Does the patient have any thyroid problems?

I wonder as to whether the deposit is a soft tissue filler or mucin. The material appears to deposit at striated muscle, hence the possibility of deep injection of soft tissue filler should be sought, despite patient's gender??

Well, first of all, congratulations to Doctor McKee by such a marvellous website. About the case, I thought in lichen mixedematosus nodular variant. There is a normal papilar dermis and brisk lymphocytic infiltrate perivascular superficial. The collagen bundles look without alterations and the fibroblast are increased.

Well done most of you got it right.
There was in fact an actinic cheilitis (the reason for the biopsy).
The mucin was noted incidentally and we signed it out suggesting the possibility
of a hyaluronic acid containing cosmetic filler. The latter was confirmed
later by the surgeon.
Regards to all
Richard
