Dysplastic naevus. Bridging nests with mild cytological atypia (the nuclei equal the nuclei of the overlying keratinocytes in size, hyperchromatic nuclei, with prominent nucleoli), stromal response (increased collagen, superficial lymphoid cells infiltrate). When there is a junctional component that is symmetrically distributed at the junctional edges that extend laterally beyond a central dermal component this forms clinically the fried egg pattern (macular component surrounding a papular centre) and histopathologically the shoulder phenomenon (not in the example shown). It has been said that junctional and compound dysplastic naevi form, clinically and histopathologically, a continuum extending from a common naevus to a superficial spreading melanoma.
