There was no sign of an origin from the surface epithelium and all the differential diagnoses were excluded with immunohistochemistry. Spindle cell AFX was the ultimate diagnosis. Please check yesterday's case for my comments.

I think without immunohistochemistry any diagnosis is a guess. The 5 possibilities, in no order, are: AFX, SCSCC, SCM, LMSC & SCA.

DD includes: Spindle cell melanoma, spindle cell squamous carcinoma, angiosarcoma, and AFX for immunohistochemistry.I think leiomysarcoma is less likely.
