As I have to go out for much of the day, I am posting the answer early. You all did very well. This is indeed a case of tumor stage MF with transformation confirmed with immunohistochemistry for CD30. The population of tumor cells are almost all blast forms. Obviously the history of prior plaques makes life much easier when approaching these images.

Diffuse infiltrate with vague nodular pattern of medium sized or large centroblast-like cells and centrocyte-like cells. Mitosis is frequent. This is a malignant lymphoid neoplasm and the differential diagnosis includes: tumour (transformation) stage of MF AND DLBCL. For initial panel of immuno (CD20, CD3, CD30, BCL2, CD21, CD23, CD30, CD4, CD8, CD7, CD5, CD10 AND BCL6)
