Male 85 years, lower legs, haemorrhagic keratotic lesions right lower inner leg. Chronic intermittent right ankle swelling. ?Kaposi's sarcoma.

Yes it is a great example of acroangiodermatitis of Mali or pseudokaposi's sarcoma. Clinically the lesions were indeed rather discreet and KS was the clinical enquiry. The clinical colleagues were quite surprised by the diangosis of a lesion related to venous hypertension. The lobular nature and thick-walled vessels are the key clues. Fibrin extravasation can also be seen in dramatic cases like this. Regards to all from Paris this week.
