Phillip's diagnosis on this case is Spitzoid melanoma. I found this case difficult on these images but based on age/site, the fairly obvious pagetoid spread and rather confluent mild cytological atypia (including the dermal component that lacks maturation and includes a rather large nest) are sufficiently disturbing to best regard the whole lesion as a superficial spreading malignant melanoma. Prognosis ought to be excellent as it is thin and lacks dermal mitotic activity. I think this sort of case will always yeild a range of opinion. Apologies for delay and regards to all.

Transepidermal elimination of melanin reflects irritation in this melanocytic lesion. Lack of upper migration and presence symmetry and well circumscription (i.e. starting and ending with nests) speaks against melanoma.

I was undecided at first, largely because of the disorderness of nests. However, the circumscription, symmetry, shouldering, nuclear monomorphism make me think this is simply a compound naevus.
