In this case the biopsy was received with a clinical diagnosis of Flegel's from an experienced dermatologist. This case clearly demonstrates without clinical help we often find ourselves lost. There is a lichenoid reaction here with some spongiosis and lost granular layer along with hyper and paraakeratosis and all these features are well described in Flegel's. Well done Mona. If this is not a lichenoid dermatitis, I honestly don't know what it is. Rook's and Fitzpatrick's both say Flegel’s disease is autosomal dominant & affects the lower legs, both of which don't fit the clinical details.

The full history should be as follows: F80 Incision biopsy, right thigh. Multiple small keratotic papules - arms and legs, 4mm, 50yrs. No family history.
