I think it is bi-phasic synovial sarcoma, for immuno to exclude other differentials as MPNST or neuroendocrine carcinoma.

I would suggest an exotic diagnosis of MCC with combined spindle cell morphology and trabecular variant, hence CK20 and synaptophysin for immunos.

Neuroendocrine carcinoma is my first choice. Obviously other small blue round cell tumours would be in the differential

Its great to see so many comments today. Most of you got the correct diagnosis which is a Merkel cell carcinoma with spindled and trabecular foci (confirmed with immunohistochemistry). Well done. Have a great wekend. Phillip
