Esinophil rich infiltrate, esinophils aligned along basal layer, in view of age and clinical picture including urticarial lesions, I will favor urticarial stage of BP.

Differential diagnosis includes: Prebullous pemphigoid, Drugs, urticaria, or due to internal malignancy. There is no evidence of vasculitis. For IF and clinico-radiological correlation. I favor Prebullous pemphigoid.

Early urticarial lesion of bullous pemphigoid. I suggest this diagnosis because there is a superficial perivascular and interstitial infiltrate of lymphocytes and numerous eosinophils. Some eosinophils line up along the dermo-epidermal junction, accompanied by mild vacuolar degeneration.

Well done. The diagnosis is pre-bullous pemphigoid. Confirmed with IMF.
