Female 73 years with multiple tumors and nodules on the right lower leg.

looks obviously malignant with the mitoses and pleomorphism. Given the history, these are multiple local mets. Regarding cell lineage (of these plump epithelioid looking cells) I would probably say melanocytic (probably due to the unilaterality of the lesions) vs lymphocytic (anaplastic, but would expect bilateral and more widespread lesions). But one would need a wide immuno panel, including S100, Melan A, HMB 45, LCA, Pan K to start with, and if all negative followed by vimentin, SMA, Desmin, CD 99, EMA etc....

The lesion shows slight epidermotropism devoid of a grenz zone.
The cells are anaplastic and very pleomorphic and therefore it is probably not cutaneous DLBCL.
It is most likely Anaplastic Large Cell Lymphoma
