As always, clinicopathological correlation. Foreign-body granulomatous reaction (macrophages, foreign-body giant cells and lymphoid cells). Gouty tophus, this is a formalin-fixed biopsy with dissolved urate crystals and amorphous pink areas corresponding to the sites of crystal deposition. In alcohol-fixed biopsy, the crystals appear brown and needle-shaped.
