Non necrotizing giant cell rich granulomatous reaction (astroid bodies are seen in the giant cells), would suggest sarcoidosis or Foreign body reaction after excluding infectious aetiology (PAS, Fite, ………). Correlation with clinical findings (chest X ray,.....) is essential.

I fully agree with the differential diagnoses of sarcoidosis and infectious conditions. The diagnosis in this case is granulomatous tenosynovitis. This is a very rare condition which most often is associated with a mycobacterial or atypical mycobacterial infection. It has also been described in association with Sporothrix schenckii and there is a report of it following trauma associated with a broken fluorescent light bulb! In this case, the cause is unknown.
