Tough!!, as is obvious from the varied responses. I thought the cells looked a bit histiocytic. There is a sort of interstitial pattern with a suggestion of necrobiosis and mucin. I am however not certain. Papular GA came to mind, but the history and the histo is not really typical. I wasn't convinced the cells were neoplastic. Looking forward to be educated!! :)

This is the sort of case that I find very difficult and the clinical information for me is critical. The bottom two figues show blast cells. This is an example of acute myeloid leukemia. When I look at this sort of case, I always try to remember leukemia cutis at the back of my mind when I consider the differential diagnosis (it doesn't always work!). Fortunately, the clinical information disclosed that the patient had AML. Hanan did very well and was closest to the correct diagnosis.
