55 years-old male. 4/7 tender lesions on knee, legs and arms. Also iritis/weight loss/headache. ?Vasculitis. ?Sarcoidosis.

Interstitial granulomatous dermatitis. Given the clinical picture, a wide range of associated systemic disorders should be excluded: collagen diseases, myelodysplastic syndrome, lymphoproliferative disorders etc...

Interstitial granulomatous infiltrate surrounding basophilic material blended with bundles of necrobiotic collagen and nuclear dust of neutrophils. That means palisaded neutrophilic and granulomatous dermatitis, a cutaneous condition seen in the setting of systemic disease, such as Sjögren syndrome, lupus erythematosus and rheumatoid arthritis.

We reported this as palisaded neutrophilic and granulomatous dermatitis.
We also commented that this reaction pattern may be associated with any number
of systemic conditions!! So far we have had no specific underlying diagnosis
made in the limited follow-up available. Well done to all and enjoy your weekends.
