53 year-old male with slowly enlarging left palm mass since 2009 (biopsy was done end of 2012). There was also an elongated foreign body removed from the left palm at the time of surgical procedure. The lesion was well-circumscribed, measuring 4.5 x 3.5 cm.

Morphology compatible with DFSP, fibrosarcomatous variant but site is unusual.

A highly cellular tumor of spindle cells arranged in fascicles with a herringbone pattern. Probably there is increased number of mitotic figures (power of the pictures doesn’t permit me to assure this finding).
I agree with my colleagues: dermatofibrosarcoma protuberans with fibrosarcomatous transformation, although in upper extremities this lesion has a strong predilection for proximal sites rather than distal ones. Other areas should have more conventional storiform pattern.
If this diagnosis confirm, the well-circumscribed limits of the tumor are merely apparent, because neoplastic cells typically infiltrates beyond grossly visible margins.

I would also include cellular schwannoma in the differential as the lesion is stated to be well-circumscribed.
