This is reaction to filler.there is a recent review by dr requena is very helpful to know the histpath of each materials used in the fillers.

Foreign body granulomatous reaction against bluish amorphous injected filler. Agree with Dr. Hurt that the most likely filler is hyaluronic acid gel.

I think Dra. Maria George is referring to [url="http://download.journals.elsevierhealth.com/pdfs/journals/0190-9622/PIIS0190962210006973.pdf"]this[/url] paper. It’s the same one that I mentioned in a comment on Case 753, on 7th May 2013.

On pressing the patient finally admitted to intra-oral injection of 'Radiesse' (Radiesse ® (BioForm Inc., Franksville, WI, USA) a "biocompatible" product that consists of calcium hydroxapetite, CaHA microspheres suspended in a carboxymethylcellulose gel. I e-mailed Dr Requena and we think there may also be separate hyaluronic acid here as suggested by Mark - well done!! I am guessing you guys see a lot more than us in the UK although I have noted an increase in the last few years.

Cosmetics patients may go for a different 'product of the week' when they get touch ups ....nice to see a case of two mixed patterns. Seems a rare occurrence (at least in my patient base).

Anyone have an opinion as to what percentage of filler biopsies show two or more different cosmetic fillers at once?
