Diffuse neutrophilic infiltrate and nuclear dust throughout dermis with accentuation around swollen-walled vessels. There’s also epidermal hyperplasia and parakeratosis with serum crusts.
Clinical presentation suggests neutrophilic dermatosis of the dorsal hands, but as I didn’t find any evidence of vasculitis, I favor Sweet’s syndrome.
Lesions like that described here, resembling superficial pyoderma gangrenosum, enhance concern about underlying leukemia, although more widespread distribution should be expected in malignancy-associated cases.

Neutrophilic dermatosis of the dorsal hands. But agree that it is in the same spectrum as Sweets and PG. Does the patient have rheumatoid arthritis?
