64 years-old white male with numerous cutaneous squamous cell carcinomas in the past. This lesion is a biopsy from “top of scalp”, came as “rule out SCC”.

malignant spindle cells in the dermis.
DD are SCC , AFX, and Melanoma.
Need IHC.

DD: Spindle cell SCC, Melanoma, AFX. Requires immunohistochemistry.

[font="Palatino Linotype, serif"]I favor myxoid spindle cell squamous-cell carcinoma, since epidermic cells seem to blend into the spindle neoplastic cells embedded in a mucinous stroma. Furthermore, there's no junctional atypical melanocytic proliferation nor bizarre cells of atypical fibroxanthoma.[/font]
[font="Palatino Linotype, serif"]Anyway, immunohistochemistry is required to achieve the correct diagnosis.[/font]

Spindle squamous cell carcinoma. The tumor cells were p63 and CK 5/6 positive (and negative for melanocytic markers and CD34).
