Eccrine spiradenocarcinoma with a precursor lesion(adenocarcinomatous change in spiradenoma).

Dual population of cells at left side of fig. 1 reminds me of eccrine spiradenocarcinoma arising on benign counterpart.

The first figure show two architecturally different patterns. The one on the left has a cylindromatous pattern the other is adenocarcinoma nos. This could be an adnenocarcinoma ex cylindroma or freakishly two separate lesions the right one being the metastasis.

The lesion is a spiradenocarcinoma with the precursor lesion on the left in Fig 1. Elsewhere there is loss of a dual cell population, mild pleomorphism, mitotic activity and necrosis.
