70 years-old male. Panniculitis left lower leg. Diag. PAN 4 years ago. New area biopsies. Recurring on reducing immunosuppression. ANCAs were negative.

I think it is nodular vasculitis, since the mixed cell infiltrate is present within arterial walls and also gives rise to an extensive lobular panniculitis.
In polyarteritis nodosa the infiltrate is expected to involve scantly the fat lobules that surround the affected artery.

I am favoring nodular vasculitis because the inflammation is extensively involving the fat lobules and not limited only to the vicinity of the involved vessel.
