Foreign-body granulomata against cutaneous implant, probably injected with cosmetic purpose.
Once (29 Jan 2013) I suggested a review about this topic. Now I’d like to suggest a more recent one:

Deposits of bluish hue with fibrosis and granulomatous tissue response suggestive of foreign body reaction to filler material.

Agree : a deep dermal polymer filler injection with impressive palisading foreign body granulomatous reaction, and fibrosis.
This lesion has some very impressive foreign body giant cells, some with multiple asteroid bodies.
Looks like this filler injection has been around for a *very* long time. As has the patient, if the marked solar elastosis in the superficial dermis is any indicator...

That is correct, it was a granulomatous reaction to hyaluronic acid filler.
