METASTATIC ADENOCARCINOMA

i will do imunos

I never in my clinical life so far saw a sebaceous cyst or epidermal cyst in a patient in his 9th decade
you need also to do TTF and CK7 and CK20 to exclude pulmonary and ccolon sources

I think this could be a metastasis from an invasive ductal carcinoma of the breast.

I will not go far away from sweat gland carcinoma, and then rule out other things

Adenocarcinoma, possessing a microacinar pattern. I'll add acinar cell carcinoma of the pancreas & endometrioid adenocarcinoma.

This was a metastasis from the colon with strong expression of CEA and CDX2. Agree breast would have been the most likely first guess.
