Hyaline thrombi and extravasation of RBCs. Vascular occlusive disease, e.g. TTP, cryoglobulinemia, etc.

Other differential diagnosis may be capillary fragility with extravasated red blood cells due to Amyloid deposits. Possible cutaneous involvement by primary systemic amyloidosis. Congo red and polirized light could help.

There was a history of cocaine use, and so this was called levamisole-induced vasculopathy.
