Four diagnoses came to my mind: acute generalized exanthematous pustulosis, acute generalized pustular psoriasis of von Zumbusch, IgA pemphigus, and subcorneal pustular dermatosis of Sneddon and Wilkinson. The presence of mild spongiosis and mixed cell infiltrate in superficial dermis make me favor the first one.

I go with acute generalized pustular psoriasis of von Zumbusch.

My first thought was AGEP, based on the age of the patient and probably using some medications.

AGEP vs. pustular psoriasis. Even if one believes that these are two separate diseases, separating the two is not possible microscopically.

My differential was the same, but clinically, this was contact dermatitis, without any pustules. So I raised the possibility of contact dermatitis with neutrophilic spongiosis and pustules.
