I was pretty clueless too! I addition to thoughts above wondered about an odd neural / ganlioneuromatous lesion. Nuchal fibroma was another thought.

I apologize for the delayed response. I am afraid the features didn't photograph as well as I would have liked. I called this a smooth muscle harmartoma.
