I'm struggling with this lesion. My differential is with giant cell fibroblastoma (although it doesn't fit my stereotyped idea of that kind of lesion) versus connective tissue nevus versus elastofibroma dorsi (although the clinical context doesn't seem to fit). Educate me, please!

I was pretty clueless too! I addition to thoughts above wondered about an odd neural / ganlioneuromatous lesion. Nuchal fibroma was another thought.

I apologize for the delayed response. I am afraid the features didn't photograph as well as I would have liked. I called this a smooth muscle harmartoma.
