45 year old male with multiple generalized violaceous and erythematous skin lesions.
Otherwise asymptomatic. Biopsy of right arm.

I see slit-like spaces and vascular and spindle cell proliferation with vacuoles containing red blood cells. Kaposi sarcoma.

Agree. Kaposi sarcoma. Also distinctive spaces dissecting the collagen bundles, RBC extravasation ... along to a concerning clinical presentation

Focally glomeruloid vessels, dermal alterations resembling nodular fasciitis. I think this is cutaneous reactive angiomatosis.

Agree with Kaposi. It was only yesterday that I saw a case of Kaposi (in a gastric biopsy).

My blinded diagnosis was diffuse dermal angiomatosis / reactive angioendotheliomatosis vs. Kaposi's sarcoma. In light of the clinical information, Kaposi's seems more likely. HHV-8 would be helpful.

In this clinical setting, microscopic images showing proliferation of jagged slit-like vascular spaces that dissect between collagen bundles and protude into a pre-existing vessel as a promontory sign make me think of Kaposi's sarcoma.

I thought KS too but would always confirm now with HHV8.

Dear friends, and colleagues in Manchester, Thank you for the opportunity to come to Manchester. The forum was amazing and I had a wonderful time. My thanks to Mark as well for his wonderful mentorship and the opportunity to participate in dermpathpro. It's been an amazing learning experience and you all are really great dermatopathologists!
