The patient is a 68 year old man with a previous biopsy from 2 years ago. The patient now has a re-excision with margin exam taken from the left aspect of the abdomen at the midline.

Yes, I diagnosed this as a persistent melanocytic nevus. I think this lesion was relatively straightforward, but many are not. I use the term "persistent" rather than "recurrent", as the lesion was never excised in the first place.

It is an important distinction, as in the so-called "recurrent" melanomas -- are they persistent primary melanomas or locally metastatic melanomas? A persistent primary melanoma and a locally metastatic melanoma have different prognoses. The term "recurrent" doesn't clarify the matter in those kinds of cases.
