Erythema multiforme or acute graft versus host disease, the latter if clinical history is compatible.

Erythema multiforme and erythema multiforme-like acute graft-versus-host disease are my hypotheses. I favor the first one since there's no vacuolar alteration of the follicular epithelium.

I had another couple of random thoughts in addition to the above. Drug reaction (high apoptotic keratinocytes), adult onset Still's (high apoptotic keratinocytes), Measles, pityriasis lichenoides.
