My first hypothesis is Interstitial granulomatous dermatitis also. I would like to add a rare possibility of Whipple´s disease (would perform PAS stain). I have a case like this in my file.

I see a slight proliferation of fibroblasts in addition to mucin deposition in the upper dermis. No sclerosis is seen. I think this is papular mucinosis (lichen myxedematosus).

Agree with the differentials given up and I believe this case needs clinical correlation.

Given the interstitial pattern and mucin, I called this interstitial granuloma annulare.
