If adequate history is obtained (cocaine user) this could be levamisole induced necrosis symdrome (LINES).

Agree with Romualdo, the association of neutropenia and widespread necrosis should raise the possibility of LINES

This patient had disseminated intravascular coagulation (DIC)

I take it the first image shows one area of vascular thrombosis at the left edge to make it DIC causing the epidermal necrosis?
