Case History: The patient is a 65 year old man with an excision of three enlarging lesions present five years taken from the scalp.

I think this is the most benign end of the spectrum of a proliferating trichilemmal cyst. The lesion is well circumscribed and has a focal proliferating area with clear outer root sheath differentiation. There is also an area were cells seem to have keratohyalin granules and limits better defined, maybe hybrid features?

-- TRICHOLEMMAL CYST (ISTHMUS-CATAGEN), FOCALLY PERFORATED,

WITH AN AREA OF PROLIFERATIVE TRICHOLEMMAL CYSTIC ACANTHOMA
