Case History: 63 year-old male with forearm lesion (biopsied February 2014). Currently, the patient has metastatic lesions in the lung and brain.

Finding this tricky without immunos. Amelanotic melanoma?

One image shows what looks like osteoid. Another possibility is extraskeletal osteogenic sarcoma.

Metaplastic amelanotic melanoma. Area with osteoid. Foci that looks angiosarcomatous.

Cutaneous carcinosarcoma will also be in the differential. Difficult without IHC.

Melanoma. Agree on performing a panel of immunos (ck, vim, s-100,desmin, cd31, cd34 and eventually hmb-45, melan A)

Epithelioid angiosaroma with osteoid.  Melanocytic markers were negative.  Positive for CD31, ERG and FLI-1.
