37 year-old HIV positive male (CD4 200) with history of Kaposi sarcoma, presenting with numerous perianal hypopigmented papules and plaques.

I think this is bowenoid papulosis, but the information that the papules and plaques are "hypopigmented" confused me.

Due to the clinical presentation with hypopigmented lesions, I prefer to call it anal intraepithelial neoplasia.

Agree with Bowenoid papulosis, in context of discrete perianal papules. Low-grade random atypia/enlargement in stratum spinosum keratinocytes, koilocytic changes (aberrant molded multinucleation or perinuclear clearing), sparse high level mitoses and sparse apoptosis, and scattered microscopic mounds of "paradoxical parakeratosis" overlying hypergranulosis (1st image).
