Hailey-Hailey variant of Grover's disease. Hailey-Hailey's disease as a second possibility, although back is not a prefered location. CPC is needed to arrive at a final diagnosis.

I would add Pemphigus vulgaris to the differential of Hailey-Hailey, and Hailey-Hailey-pattern Grover's disease (as we are absent clinical history or lesion pattern, other than location). There is 'tombstone sign' in the intact basal keratinocytes and suprabasal clefting. That said, the absence of adnexal involvement would give favor to Hailey-Hailey or Grover's dz. Positive DIF/IIF would support pemphigus.
