Nodules of epithelioid and spindle myofibroblasts within dense collagenized fascial tissue: palmar fibromatosis is my first thought.

I favor a plexiform benign nerve sheath tumor. Epithelioid schwannoma?

My first thought was plexiform fibrohistiocytic tumour but I like the idea of ruling in / out plexiform epithelioid schwannoma.

S-100 and CD34 were negative. There was no ossification in this lesion. I called this diffuse tenosynovitis.
