There appears to be a leukocytoclastic vasculitis component with focal necrosis; would add local hypersensitivity Type III reaction (Arthus reaction) to the differential, depending on the clinical context. Arthus reactions have been infrequently reported after vaccinations containing diphtheria and tetanus toxoid.

Epidermis is necrotic and keratinocytes show intracytoplasmatic eosinophilic inclusions suggestive of Guarnieri’s bodies. Local vaccinia vaccination site reaction is my choice.

I think I interpreted image 3 wrong...
I re-examined the photos again
Would like to suggest now: chicken pox like rash at site of varicella vaccine...

How annoying as I thought these were Guarneri bodies and even tried to look up [u][b]live[/b][/u]pox virus vaccines and could not find any in current usage. I did not think to go back in history!!!!
