?BCC with plasmacytoid/myoepitheial differentiation.
I can see distinct areas showing eosinophilic hyalin inclusions with eccentric nuclei in a background of more typical BCC

My suggestion is basal cell carcinoma with myoepithelial differentiation (plasmacytoid cell variant with hyaline inclusions).

My first thought is BCC with myoepithelial differentiation or rhabdoid BCC.

My diagnosis was:

SKIN, LEFT MID BACK , SHAVE BIOPSY :
[b]-- BASAL CELL CARCINOMA WITH MYOEPITHELIAL DIFFERENTIATION[/b]
[indent=1][b]COMMENT: [/b]This lesion seems to fit the profile for basal cell carcinoma with myoepithelial differentiation, which has been described in a few articles in the literature. I have referenced one of the more recent articles below. The present lesion seems to be commensurate with those published previously.[/indent]
